Who do you ask to speak to and how do you go about it? In person? Do you get applicants first or clients?

You get clients first. Initially you'll end up with companies who will be trying you out in competition with other companies for 25-30% of the salary of the person they're trying to hire and you then have to go find them on your own dime. So it's a money loser to start, but if you can place a few, you'll get some breathing space -- and some success stories -- so you can start trying to land exclusives.
